A.C.T. Program Lead Clinical Therapist, Team Wellness Center

  • Directed the Assertive Community Treatment Team assisting in the provision of treatment, rehabilitation, and support services to identified clients with severe and persistent mental illness.

  • Was tasked with revamping and restructuring team in order to bring program into compliance; program had previously failed county audit 3 years consecutively, after 1 year in position was able to have program successfully pass county/state audit with evaluation score of 91%.

  • Supervised and evaluated the multidisciplinary team; met regularly with other ACT staff.

  • Conducted initial intakes; directed and coordinated the client admission process, assignments of the most appropriate staff, clinical treatment and additional support services.

  • Provided population and age-specific individual/family therapy, as well as provided clinical management of all clients in crisis and facilitated psychiatric hospitalization.

  • Led daily treatment team meetings; continuously evaluating the status of clients and conducting appropriate coordination of treatment to ensure immediate attention to their changing needs.

Case Manager, Ulliance HPRP (Health Professional Recovery Program)

Troy, MI (July 2016 – October 2016)

  • Managed a case load of licensed professionals who were participating in a state contracted health monitoring program for a mental health, substance abuse or dual diagnosis condition.  Interacted with all licensees, PHM providers, state and licensing board personnel in a professional and helpful manner.

  • Monitored licensee’s compliance with monitoring agreements in areas such as treatment recommendations, attendance in individual, support, and group therapy programs, drug testing/monitoring programs, employment rules and restrictions and reporting requirements.

  • Reviewed daily drug testing results, monthly self-reports, and addressed all non-compliance behaviors of Licensees which included presentation of non-compliance issues in weekly team review meetings. Was responsible for carrying out and communicating team decisions with Licensees and their health-care providers (therapist, psychiatrist, addictionist, and other professionals such as pain specialist). Additionally, prepared quarterly reports, vacation requests, worksite monitoring agreement addendums and all other required documentation for state and licensing boards.

I just wanted to take a moment to address my sincerest appreciation for your center and what it has done for my son. In particular, his time with Summer. 

 

Initially, I was pensive about his therapist changing since he had formed such a good rapport with his last therapist. Once I met Summer and realized her objectives-that fear subsided. Since my son has started therapy with Summer, he has opened up and made great strides regarding expressing his emotions in a positive and healthy way. Each week, she works with him on how important his choices are as well as the importance of recognizing that his voice matters and should be heard. It's been amazing to watch the progress! My son has a lot of pain in his life, as a parent, I am doing my best to learn how to not contribute to that along with identifying what I can do to facilitate healthy emotional growth and assuage him when he is in need of emotional support. Working with Summer, I have learned how to do that effectively in order to have a healthy balance of communication with my child.

 

I know that you guys work very hard to ensure that the kids will have the best possible therapists to work with; Summer by far surpassed all of my expectations in a therapist.
